About our lab dockets

Before submitting your case, please, fill out a lab docket. It is important to give us as much information as possible to ensure communication quick, simple and product delivery on time.

For your convenience we made our Digital Lab Docket which is responsive and easy to fill out online and save it on your computer. Then you can choose to send the docket with your digital impression via email or with larger files via We Transfer.

Also, you can print out the Lab Docket and send us with traditional impression by post. If you would like a Lab Docket booklet please order HERE and we will be happy to send it to you as soon as possible.

Send a digital impression

We are accepting digital files (STL) from all digital intraoral scanners. You can send us case:

-via email (up to 20mb)

-via We Transfer ( larger files )

Send a traditional impression

Please ensure that impressions, lab docket and all other enclosures are carefully packet for safe delivery to our laboratory.

For your convenience we will organize and cover delivery costs.

Contact us to arrange pick up and we will fill out label info and send you label to print in .pdf form.
